CHICAGO (Sun-Times Media Wire) -

A man is dead and a boy was injured Sunday when a pickup truck rear-ended two vehicles before fleeing in the Humboldt Park neighborhood.

A silver four-door pickup truck, believed to be a Toyota Tacoma, rear-ended a 2005 Hyundai Elantra in the 1600 block of North Kostner Avenue at 5:20 a.m., police said.

The pickup truck then rear-ended a Toyota Camry, police said.

The 25-year-old driver of the Toyota Camry was dead at the scene, police said. He was identified as Eduardo Perez of the 1500 block of South Clarence Ave. in Berwyn, the Cook County Medical Examiner's office said.

A passenger in the Camry, a 17-year-old boy, was taken in "stable" condition at Advocate Illinois Masonic Medical Center, police said.

The driver and a passenger of the Hyundai, two 31-year-old women, refused medical treatment, police said.

The driver of the pickup truck fled the scene, police said.

The police Major Accident Investigations Unit is investigating.
