Elderly man with dementia missing from Humboldt Park

CHICAGO (Sun-Times Media Wire) -
Police are looking for an 82-year-old man who suffers from dementia and has been missing from his Humboldt Park home since Wednesday morning.
Ismael Quinones was last seen when he left his home in the 2000 block of North Mozart Avenue about 7:30 a.m., a missing person alert said from police said.
He was supposed to going to a store at Armitage and Mozart, but has not returned home as of about 2 p.m.
Quinones suffers from mild dementia and speaks English. He was last seen wearing a blue hooded sweatshirt, dark green flannel shirt, and tan pants. He is 5-foot-7 and 155 pounds with brown eyes and black/gray hair.
He also went missing on Nov. 5, and was found later that day walking in the Logan Square area.
